Well now we can't assume that she is smoking. Maybe someone walked by and asked her to just hold it while they were tying their shoes out of the frame of the picture. Since she is so concerned about the "noise", then being aware of all possible health concerns to her unborn would make her never touch a cigarette. Parents always put their children's needs in front of their own.
